The FY2 will be part of the Acute Medicine team looking after acute admissions and patients in ambulatory
care. Trainees are supported by daily ward rounds of the consultant acute physicians.
The Acute Medicine firm is (kind of) famous for our research output - from working with world leading
technology companies to hosting the national audit in acute medicine to testing new ways of working with
patients to deliver better care.
We love having trainees and hope that the FY2 will be willing to share their knowledge with the FY1s, the 3rd, 4th and 5th year medical students and the Physician Associate students that we are regularly our guests.

Rotation 2
Renal Medicine

What type of work to expect and learning opportunities:
During your time with us you will:
• Learn how to assess and manage AKI • Renal pharmacy including immunosuppressants and nephrotoxic medications
• Opportunities to participate in clinical audit • Learn how to manage CKD complications
• Observe and potentially perform a temporary dialysis line insertion • Basic principles of HD/PD/transplant
• Learn how to manage medical emergencies in general medicine
Your Main Duties:
• Daily inpatient care for general medicine and nephrology • Arrange investigations • Discussions with relatives

General Surgery

What type of work to expect and learning opportunities:
The Department of General and Colorectal Surgery comprises of 8 consultants. There are 4 F1s in the
department, each of whom is allocated to a team of two consultants. During this rotation you will gain
experience in the management of common acute and elective General and Colorectal conditions. The
placement will allow you to develop the majority of the competencies required in the F1 curriculum.
Clinical duties are as follows:
• Initial assessment and management of emergency surgical admissions
• Maintaining hospital notes and drug charts
• Prescribing drugs and IV fluids
• Requesting referrals to other services
• Exposure to surgery in the operating theatre
• Initial assessment and management of ward patients who become unwell
• Assisting with ward rounds
• Preparing discharge summaries
• Preparing patients for surgery
• Participation in audit and morbidity /mortality presentations
• Communication with patients and their relatives
• Requesting investigations, documenting results and acting upon them
• Team working
• Fulfil requirements of the placement e.g. CBD, mini CEX, DOPs
Your typical working pattern in this placement could be:
You will participate in a 1 in 8 on-call day time rota with your own team and remain with the same team to
support post-take ward work. This allows you to experience the whole patient journey while managing
common acute surgical conditions. Once every 4 weeks, you will be allocated time to join the operating
theatres, endoscopy and clinics with your clinical supervisor.
There are no night duties and this post attracts a 1B banding.

Geriatric Medicine (COTE) F2

In Care of the Elderly medicine, we care for older people with a wide variety of acute medical presentations, as well as the ‘geriatric giants’ of falls, frailty and delirium. Two of our consultants sub-specialise in Movement Disorders, and our foundation doctors will therefore also gain experience in the management of people living with Parkinson’s and related conditions. We also have an acute stroke unit, and when on this ward, you will also have the opportunity to learn about Thrombolysis in stroke, Thrombectomy and treatments of acute TIA, complications of stroke, principles of rehabilitation and begin to interpret CT images.
Where possible, we aim to give our foundation doctors the opportunity to spend time on both our general Care of the Elderly Ward (Glaslyn) as well as our stroke ward (Prysor). Multi-disciplinary team working is a vital component of medicine for older adults, and you will experience being part of the MDT. You will also form part of the acute medical on call rota during this post, providing you with experience in a wide range of unselected acute medical presentations.

Your typical duties during this placement will include:
• Taking part in Consultant and SpR led ward rounds.
• Responsibility for ongoing care of inpatients in between senior ward rounds, with support of the rest of the team as required.
• Communication with patients and their carers.
• Take an active role in the MDT, liaising and working with other professions (such as nursing, therapy and social work) to provide holistic care to our patients
• Attend weekly department teaching, as well as the organised foundation teaching.
• On call duties as per the medical rota.
• You may also wish to take the opportunity to attend specialty clinics when staffing levels allow.

Respiratory Medicine

During your time with us you will work as part of a friendly team on a busy General Internal Medicine and Respiratory ward. You will learn about the management of acute and chronic respiratory diseases in a supportive training environment.

Rotation 3
Community Palliative Care

What type of work to expect and learning opportunities:
During your 4 month rotation, you will gain experience in working within a multidisciplinary team in a community setting, to support patients and families in their own homes. You will be based in the community specialist palliative care team office on the Ysbyty Eryri site in Caernarfon and work closely with our specialist nursing teams, GPs and district nurses and the senior doctors in the specialist palliative care team, to deliver palliative and end of life care in the community setting.
You will gain experience in managing symptom control, end of life care and psychosocial support to patients with life limiting illnesses (both cancer and non-cancer). You will learn how to carry out holistic assessments, extend your clinical skills in an environment without ready access to investigations and develop your communication skills and confidence in discussions with patient about their choices and wishes for their future care.
By the end of the placement, you will have improved your knowledge, skills and confidence in palliative and end of life care, including when to refer to specialists within the hospital for interventions that may improve quality of life or symptom management. You will be encouraged to carry out a piece of audit or quality improvement work during the placement.
This post also has additional option to carry out an educational postgraduate diploma over the Academic year during all three foundation posts – 1 day a week available for academic studies.
Your Main Duties:
Providing general medical support to our specialist nursing team during home visits, by way of clinical assessments and physical examinations. Ensuring appropriate prescriptions are available for anticipatory prescribing and managing symptoms in the last days of life through discussion and liaison with nurses and GPs. Attending weekly MDT. You must be able to drive and have your own transport (including insurance for business use for the duration of the placement). Travel expenses for community visits will be reimbursed as appropriate according to usual BCU policies.

Daily team triage / handover meeting to discuss / allocate new referrals and plan for the day.
